Registration for the Etiquette class (ages 4-6) is a 2-step process.

1. Submit a registration form to PRUMC Sports & Recreation.

2. Print, complete, & submit the form below directly to your child's teacher at least one day prior to the first etiquette class. Your child will not be picked up from the PRUMC Preschool unless this form has been turned in.

I understand that it is my responsibility to notify both the Preschool and the Recreation Department on the days when my child will not be able to attend these classes.
