Registration for the Tot Shots class is a 2-step process.

1. Submit a registration form to PRUMC Sports & Recreation.

2. Print, complete, & submit the form below directly to your child's teacher at least one day prior to the first Tot Shots class. Your child will not be picked up from the PRUMC Preschool unless this form has been turned in.

Students will be picked up from the carpool line by a member of Recreation’s staff and taken to the class.  Please make sure that my child is included with the group going to Tot Shots on this day. 

I understand that it is my responsibility to notify both the Preschool and the Recreation Department on the days when my child will not be able to attend these classes.
