Waiver of Liability and Release

The undersigned parent or legal guardian of the child(ren) named below has enrolled that child(ren) to participate in camps; programs and recreational activities facilitated through Peachtree Road United Methodist Church or affiliated entities (collectively PRUMC). PRUMC will accept the child(ren) for participation only if the parent or legal guardian receives a copy of this statement, reads the entire statement, accepts certain responsibilities for him or her self and/or the child(ren), and releases all persons and parties through whom the programs are facilitated from liabilities under most circumstances. Signature below is required for participation in PRUMC’s programs.

By signing this document, the parent or legal guardian confirms that he or she has authority to sign, has read the entire document and has understanding that the document waives certain rights of the child(ren) and the person signing.

The parent or legal guardian acknowledges that the child(ren)’s participation is voluntary and not required in connection with church membership or attendance. The undersigned further acknowledges that participation in swimming, athletics, hiking and other forms of physical recreation activities can or may result in physical injury. While PRUMC intends to operate its programs in a reasonable manner, the undersigned acknowledges that many of the activities in which the child(ren) will participate will involve physical activities, which inherently may result in physical injury even if the program is conducted with appropriate levels of care and supervision.

The undersigned expressly acknowledges that neither PRUMC nor any of its officers, board members, directors, employees, agents or other representatives (collectively Representatives) can be responsible for any injuries to persons or property arising out of participation of the child(ren) in the programs. In consideration of the benefits to be derived by the child(ren) and the undersigned from participation by the child(ren) and any other person who may have any rights to assert any claim on behalf of himself or herself or the child(ren) with respect to injuries to the person or property of the child(ren), hereby waives and releases any claims for damages against PRUMC and/or its Representatives arising out of injury or damage to the person or property of the child(ren) and hereby releases and forever discharges PRUMC and its Representatives from all actions, causes of action, suits, damages, judgments, claims and guardians, representatives, heirs, executors, administrators, successors or assigns may have against PRUMC and/or its Representatives arising out of participation by the child(ren) in programs facilitated by PRUMC.
